Steps to Gain an Athletic Clearance

Step 1

·       Fill out the White MUHSD Athletic Participation Form-

o  Dr. Signs the bottom

Step 2

·       Return both forms completed with parent signatures and up to date insurance information to the Student Body Office. [Front and Back require signatures.]

Step 3

·       Your Physical form and Athletic Participation form will be reviewed and checked against the ineligible and obligation lists.

·       Your head coach will receive your clearance by Monday  August 6^th.

·       Your coach must have a copy of your clearance before you may participate in practices/tryouts on August 6th
