Required Enrollment Information and Transcript Request
1. Adult Parent/Legal Guardian must be present when enrolling student. A Caregiver Form from DISC Office is required if student lives with some other than a parent. (DISC office is located at 3105 "G" Street on the corner of Olive and G Streets.
2. Proof of residency is required. A current PG&E utility bill, Water and Garbage bill can be used. If recently moved and rent, you may bring in your Rental Agreement signed by the landlord or rental manager. Any other documents must be approved by the Child and Welfare Department, Kelly Bentz, Administrator located at the DISC Office.
3. Transcript/Drop Slip (withdrawal grades) from previous school.
4. Immunization Records
If you do not have items Transcript/Drop Slip (withdrawal grades) from previous school and Immunization Records, please call your former school and have them fax this information to us.
