School Enrollment Process
1.  Parent/Legal Guardian must be present when enrolling students.  If the student lives with someone other than a parent a Caregiver Form is required.  The Caregiver Form must be obtained from the DISC Office which is located at 3105 'G' Street (the corner of Olive Avenue and 'G' Streets).
2.  You must bring proof of residency such as a current PG & E bill or water/garbage bill.  If you recently moved and rent, you may bring in your Rental Agreement.  Any other documents must be approved by the Child Welfare & Attendance Department, Kelly Bentz, Administrator.  That office is also located at the DISC (see above).
3.  Bring a transcript or drop slip with withdrawal grades from the previous school.
4.  Bring an Immunization Record.
