This form must be completed by student, teacher and parent before a student can become a TA. The same form is used to be an office assistant or a PAL. After you have completed the form submit it to your counselor. Students can only be a TA, Office Assisstant, PAL or AVID tutor one time per year. You must be in the 11th or 12th grade and you must pass both parts of the CAHSEE.
This form must be completed prior to enrolling in any Merced College course. Please look at the course catalog on the Merced College website to select a course that will work for you then bring this form to your counselor with the class title and # of units. Once you have completed the top portion of the form and your counselor has completed their portion you will also need the signature of a Merced College counselor and/or dean before you can register in a class. Students must submit this completed form along with their transcript to Mr. Callihan in H15 for membership in CSF. You should also refer to the CSF List of Eligible Courses below to complete the application. Membership must be applied for every semester to remain a club member. Applications are typically collected in September and February. Listen to the morning announcements for more details on when applications will be due.

AVID is a class and a program available to 9th through 12th graders. This brochure gives more information on the program and what it offers to students.
Adult School is available to 12th grade students and in some situations 11th grade students. Students can only take classes that they have previously failed and need to make up for graduation. Students are typically able to complete 5 credits during the course of a semester. This flyer gives information on the program and it's policies but students must see their counselor for an enrollment form.
